Abide Health Services respects the privacy of the members, caregivers, providers, and partners we serve. This policy explains what information we collect through this website and how we handle personal and health information.

Information we collect

  • Information you give us: name, organization, role, email, phone, and anything you write in the contact or referral message field.
  • Technical information: basic device, browser, and usage data collected automatically when you visit the site.

Please do not include detailed clinical information in the website form. Our team will collect what is clinically necessary through secure, HIPAA-appropriate channels.

How we use information

  • Respond to inquiries, referrals, and contracting requests.
  • Coordinate equipment orders, authorizations, delivery, and follow-up.
  • Verify eligibility and bill the responsible payer.
  • Improve our website, service quality, and operations.
  • Meet legal, regulatory, accreditation, and payer requirements.

We do not sell personal information, and we do not sell or rent health information.

Protected health information and HIPAA

When we function as a HIPAA-covered supplier, protected health information (PHI) is handled under the HIPAA Privacy and Security Rules. We use and disclose PHI for treatment, payment, and health care operations, and otherwise as permitted or required by law.   • Ordering physicians, case managers, and authorized representatives involved in care.

How we protect information

We use administrative, physical, and technical safeguards — including access controls, encryption in transit, and workforce privacy training — designed to protect the information we hold. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

Retention

We retain records for as long as needed to provide services and to satisfy legal, payer, and accreditation retention requirements, then dispose of them securely.

Your rights

  • Request access to or a copy of your health records.
  • Request an amendment to information you believe is incorrect.
  • Request an accounting of certain disclosures.
  • Request confidential communications or restrictions on certain uses.
  • File a privacy complaint with us or with the U.S. Department of Health and Human Services without retaliation.

Children's privacy

This website is not directed to children. When we serve pediatric members, information is provided and managed by a parent, guardian, or authorized representative.
